Harbor Health’s Elder Service Plan (ESP) is a Program of All-Inclusive Care for the Elderly (PACE). PACE is a health plan for older adults that brings together healthcare, support at home, and community services all under one plan.
We help with doctor’s visits, medicine, rides, meals, and daily needs so you can stay healthy while living in your community.
Our PACE plan includes all Medicare and Medicaid covered services. It also includes any other services your team determines are necessary to improve and maintain your health. PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
A team built around YOU
All PACE participants have an Interdisciplinary Team (IDT) made up of caring clinical and service professionals who work with you and your loved ones to help make living at home easier.
This team includes doctors, nurses, social workers, therapists, dietitians, and more. Your care team meets regularly to make sure all your needs are covered — from health and medicine to meals, rides, and emotional support.

- Prescription Medications Delivered to Your Home
- Visiting Registered Nurses and Home Care
- Transportation for Services Ordered By Your Team and the PACE Day Center

- Palliative and End-of-Life Care
- Physical, Occupational, Recreational, and Speech Therapy
- Behavioral Health Services and Social Work
