Elder Service Plan is a Massachusetts Program of All-Inclusive Care for the Elderly (PACE). Our program is designed to reduce stress, save time, and provide peace of mind through coordinated services and compassionate support.

Comprehensive care coordination
All medical, social, and supportive services are managed by a dedicated interdisciplinary team (IDT). This support means fewer phone calls, appointments, and worries for caregivers juggling complex care needs. ESP does the coordinating for you.
Patient priorities care model
Every decision starts with what matters most to your loved one, their goals, values, and preferences. This person-centered approach leads to more meaningful care and a better quality of life.
Helping loved ones stay at home
Harbor Health’s ESP plan is built to help older adults remain safe in their homes and communities. Our plan regularly assesses a participant’s needs for structure and support to ensure quality of life and security.
Elder Service Plan day center
Participants receive healthcare, meals, social interaction, and therapy all in one location. While your loved one is engaged and supported, you get valuable time for work, rest, or other responsibilities.
Home-based support
From skilled nursing to personal care aides, Elder Service Plan brings essential services right into the home. Having access to these services means you’re not alone when hands-on help is needed.
Interdisciplinary team approach
You’ll have access to a team of doctors, nurses, social workers, and other professionals who know your loved one and work with you to solve problems and provide guidance.
Focus on prevention and wellness
Regular screenings, wellness programs, and chronic disease management help participants stay healthier and avoid hospitalizations—lessening emergency stress for caregivers.
Covered by Medicaid and Medicare
Elder Service Plan provides participants with services or benefits that are covered under Medicaid or Medicare and authorized by their care team. This coverage helps reduce the financial strain of fragmented care or out-of-pocket costs for participants and/or their caregivers.
Counseling and emotional support
Elder Service Plan offers support not just to participants but also to caregivers—whether it’s stress management, grief counseling, respite care, or help navigating difficult decisions.

Harbor Health PACE has achieved special recognition as an Age-Friendly Health System — Committed to Care Excellence from the Institute for Healthcare Improvement in our ongoing efforts to ensure age-friendly care for all older adults we serve.
Harbor Health’s Elder Service Plan (ESP) will be your sole provider for primary care and specialty care (other than emergency services). Care is provided directly by ESP or a provider we approve.
