Comprehensive care coordination

All medical, social, and supportive services are managed by a dedicated interdisciplinary team (IDT). This support means fewer phone calls, appointments, and worries for caregivers juggling complex care needs. Harbor Health’s PACE plan does the coordinating for you.

Every decision starts with what matters most to your loved one, their goals, values, and preferences. This person-centered approach leads to more meaningful care and a better quality of life.

Harbor Health’s PACE plan is built to help older adults remain safe in their homes and communities. Our PACE plan regularly assesses a participant’s needs for structure and support to ensure quality of life and security.

Participants receive healthcare, meals, social interaction, and therapy all in one location. While your loved one is engaged and supported, you get valuable time for work, rest, or other responsibilities.

From skilled nursing to personal care aides, Harbor Health’s PACE plan brings essential services right into the home. Having access to these services means you’re not alone when hands-on help is needed.

You’ll have access to a team of doctors, nurses, social workers, and other professionals who know your loved one and work with you to solve problems and provide guidance.

Regular screenings, wellness programs, and chronic disease management help participants stay healthier and avoid hospitalizations—lessening emergency stress for caregivers.

 

Our PACE plan provides our participants with services or benefits that are covered under Medicaid or Medicare and authorized by their care team. This coverage helps reduce the financial strain of fragmented care or out-of-pocket costs for participants and/or their caregivers.

Harbor Health’s PACE plan offers support not just to participants but also to caregivers—whether it’s stress management, grief counseling, respite care, or help navigating difficult decisions.

Elder Service Plan Referral Form

Harbor Health’s Elder Service Plan (ESP) will be your sole provider for primary care and specialty care (other than emergency services). Care is provided directly by ESP or a provider we approve.
