Care is provided in the home, at one of our Elder Service Plan day centers in Brockton or Mattapan, or at one of our specialty provider’s offices.
Elder Service Plan provides any services Medicare and Medicaid cover and services deemed necessary by your interdisciplinary team to improve your health and wellbeing. Elder Service Plan guarantees access to services, but not to a specific provider.
