Am I eligible for Elder Service Plan?
You may be eligible to enroll in Harbor Health’s Elder Service Plan if you meet the following criteria.
Age 55+ years
Be certified as eligible for nursing facility level of care by the state
Can live safely in the community

Safely In the Community: To be able to meet the criteria defined by the State administering agency’s (SAA) specified in the Program Agreement:
- Level of care required to meet on-going medical, health, and social needs in the community.
- Safety of the home environment.
- Cognitive capacity of the individual as to the degree that they can be left unsupervised without jeopardizing health and/or safety.
- The willingness of the potential ESP enrollee and the family to comply with the plan of care and safety recommendations of the Interdisciplinary team.

*As of January 2019, 67% of seniors 55 and older with complex care needs, who could benefit from the services and supports Programs of All-Inclusive Care for the Elderly (PACE®) provide, did not have access to PACE. With the Support of The John A. Hartford Foundation,West Health, and The Commonwealth Fund, the National PACE Association (NPA) conducted analysis to identify the clinically and financially eligible population that could benefit from access to PACE.
Harbor Health Elder Service Plan (ESP) will be your sole provider for primary care and specialty care (other than emergency services). Care is provided directly by ESP or a provider we approve.
