Harbor Health’s Elder Service Plan (ESP) is a Program of All-Inclusive Care for the Elderly (PACE). ESP helps older adults with complex medical needs live safely and independently at home.

Complete personalized care
Our enrollment team can explain who is eligible for our PACE plan. We can also explain the services and benefits the plan provides. If you decide to join our plan, we will walk you through the enrollment process. We will explain the Harbor Health’s PACE plan will be your sole provider for primary and specialty care (other than emergency services). Learn more about who can get PACE services

Team based support
Each ESP participant is supported by a dedicated team of professional doctors, nurses, social workers, therapists, and more. We work together to create and adjust your care plans as your or your caregiver’s needs change.

Welcoming day centers
Our ESP day health centers in Brockton and Mattapan are places where participants can see their care team, enjoy meals, take part in therapy, and connect with others through social activities.

Care at home
For those who need it, we provide services right at home—such as personal care, home health visits, and transportation to medical appointments or the day center. We will also deliver your medications right to your door step!

Care coordination
Care and services are organized through one team, so everything from medications to medical appointments with specialists are well-managed. This support makes it easier for participants, caregivers, and families to coordinate care.

The people who work at ESP are at the heart of our program. They are kind, caring, and focused on helping older adults remain independent and living safely within their community.
We follow the Patient Priorities Care Model
and focus on what matters most to each participant.
Our goal is to improve overall well-being by making sure every part of care aligns with the individual’s personal goals, values, and preferences.
We understand that every person is different, and we work closely with each participant to create a care plan that supports their choices, respects their dignity, and helps them live life on their terms.
Please Note: Upon enrollment, participants must receive all healthcare, primary care, and specialist physician services other than emergency services as authorized by Harbor Health’s Elder Service Plan or be fully and personally liable for costs of unauthorized services.
