Harbor Health Program of All-Inclusive Care for the Elderly (PACE) helps older adults with complex medical needs live safely and independently at home.
PACE offers a complete package of care and support all in one program. Here’s how it works…

Complete personalized care
Caregivers should carefully evaluate the services, benefits, and eligibility criteria of Harbor PACE to determine if it alings with their goals and needs of their loved ones. Applicants should be comfortable moving their primary care to our Harbor Health PACE team.

Team based support
Each PACE participant is supported by a dedicated team of professional doctors, nurses, social workers, therapists, and more. We work together to create and adjust care plans as needs change.

Welcoming day centers
Our day health centers, located at Harbor Health PACE medical centers are places where participants can see their care team, enjoy meals, take part in therapy, and connect with others through social activities.

Care at home
For those who need it, we provide services right at home—such as personal care, home health visits, and transportation to medical appointments or the day center.

Care Coordination
All care and services are organized through one team, so everything from medications to medical appointments is well-managed, reducing stress for participants, caregivers, and families.

Dedicated staff
The people who work at PACE are at the heart of our program. They are kind, caring, and focused on helping older adults.
We follow the Patient Priorities Care Model
and focus on what matters most to each participant.
Our goal is to improve overall well-being by making sure every part of care aligns with the individual’s personal goals, values, and preferences.
We understand that every person is different, and we work closely with each participant to create a care plan that supports their choices, respects their dignity, and helps them live life on their terms.
Please Note: Upon enrollment, participants must receive all healthcare, primary care, and specialist physician services other than emergency services as authorized by PACE or be fully and personally liable for costs of unauthorized services.
