The Harbor Health Program of All-Inclusive Care for the Elderly (Harbor Health PACE) is a comprehensive and integrated model of care designed to provide a range of medical, social, and supportive services to older adults with complex medical needs. The PACE model coordinates and delivers all medically needed preventive, primary, acute, and home care services.

Harbor Health PACE practices the Patient Priorities Care Model

The goal of Patient Priorities Care is to enhance the overall well-being and satisfaction of participants by aligning healthcare services with their individual preferences, goals, and values. This approach recognizes that each patient is unique and seeks to create a collaborative, person-centered care experience that respects their autonomy and dignity.

KEY FEATURES OF THE PACE MODEL INCLUDE:

Comprehensive Care: Harbor Health PACE offers a full spectrum of healthcare, social, and supportive services, including medical care, prescription drugs, rehabilitation, personal care, and social services. These services are tailored to the individual needs of each participant.

Interdisciplinary Team- Harbor Health PACE participants are cared for by an interdisciplinary team of healthcare professionals, including doctors, nurses, social workers, therapists, and other specialists. This team works together to create and adjust care plans based on each participant’s evolving needs.

Day Health Centers – Harbor Health PACE operates day health centers, located at the medical clinic, where participants can receive medical care, social interaction, meals, therapy, and other services. These centers also serve as hubs for participants to engage in activities and interact with their peers.

Home Care Services – Harbor Health PACE provides home-based services, including home healthcare, personal care, and transportation to and from the day center and for medical appointments.

Coordination of Services – All aspects of care are coordinated, ensuring that medical treatments, medications, therapies, and other services are well-integrated and appropriate for each participant’s needs

We focus on delivering comprehensive, high-quality care to underserved populations. Harbor Health PACE is a program of Harbor Health Servcies, Inc. Harbor Health PACE serves older adults, age 55 plus with complex medical needs so they can remain in the safely in the community.

PACE offers a comprehensive and integrated model of care that can provide numerous benefits to both the caregivers and older adults.

Caregivers should carefully evaluate the services, benefits, and eligibility criteria of Harbor PACE to determine if it alings with their goals and needs of their loved ones. Applicants should be comfortable moving their primary care to our Harbor Health PACE team.

Harbor Health PACE provides a holistic and person-centered healthcare approach for our participants, focusing on maintaining participants’ quality of life and independence while delivering the necessary healthcare support services. Applicants should be moving to, or living in, the service area.
