This eliminates the need for caregivers to manage multiple appointments and services from different providers, making care more streamlined and efficient.

• Patient Priorities Care Model: The Patient Priorities Care model helps patients and clinicians focus all decision-making and healthcare on what matters most: patients’ own health priorities. This can lead to better overall quality of life for the participants.

• Avoiding Nursing Home Placement: Harbor Health PACE is designed to help older adults continue living in their communities rather than moving to nursing homes or institutional settings. Caregivers who want to keep their loved ones at home or in a familiar environment might choose PACE to support this goal.
