To join Harbor Health PACE an applicant should be moving to or living in the service area.
Harbor Health PACE also collaborates local adult day centers as part of our effort to provide comprehensive services close to home. This collaboration allows our participants to receive a variety of services close nearby, fostering convenience and accessibility while enhancing their overall well-being.
