Harbor Health PACE delivers high-quality, personalized care to help older adults remain independent, active, and safe in their own homes and communities. By bringing together medical care, social support, and daily assistance, we reduce the need for hospitalizations and nursing home stays—giving participants and their families peace of mind.
To enroll in Harbor Health PACE, you or your loved one must live in or be moving into our service area. This ensures our care team can provide in-home services, transportation, and access to our PACE Day Centers.

Community Partners
