Our behavioral health team cares for children, teens, adults, and elders who are primary care patients at Harbor Health community health centers. We are here to help our patients with a variety of relationship and mental health issues.
Integrated behavioral health
Our behavioral health providers work with our primary care medical providers. We support patients who have medical needs that are affected by social and/or behavioral concerns.
Our team consults with medical providers and offers direct services to patients. Our services include:
- Assessments
- Brief interventions
- Coaching
- Screenings
- Follow-up visits during medical appointments
- Recommendations and referrals to long-term services
- Consultations
- Psychiatric services
