Harbor Health’s Elder Service Plan (ESP) is a Program of All-Inclusive Care for the Elderly (PACE). Our PACE plan provides coordinated, compassionate care for older adults with complex medical needs who wish to remain independent at home and in their community.

We encourage you to review our services and eligibility requirements to see if ESP is the right fit for you or your loved one.

Participants receive their primary care and all covered services from ESP, or we will arrange care with one of our approved specialists.

Who Is Eligible For PACE?

You may qualify for Harbor Health’s PACE plan if you meet all the following criteria:

  • Age 55 or older.
  • Live in or are moving into our service area. For a listing of zip codes, click here or view the service map below.
  • Certified by the State of Massachusetts as needing a nursing home level of care – we’ll help complete this assessment on your behalf.
  • Can live safely at home with the support of ESP services.
  • Agree to receive care only through Harbor Health’s ESP, except in emergencies or pre-approved situations.

How do I get started with ESP?

Guiding you every step of the way.

Enrolling in ESP is a collaborative process designed to ensure that you or your loved one is eligible and well-suited to benefit from our comprehensive services. From your first call to the start of care, our team will be with you every step of the way.

Here's what the process looks like...
Step 1: Conversations with our enrollment and care teams.

We’ll talk with you and your care partners to explain what our plan offers. We’ll ask for information like your age, address, current health con includes reviewing your age, address, current health conditions, and existing care plans. This information helps us begin shaping your personalized PACE care plan.

A Harbor Health ESP clinician will visit your home to better understand your medical, daily living, and social needs. The clinician will determine if you can stay safely in the community with support from our plan. Other ESP team members may also do additional assessments. We’ll also invite you to visit the Harbor Health ESP Day Center so you can get to know our program and supportive environment.

Step 3: Enrollment Support

If you’re eligible and want to join our PACE plan, our enrollment team will help you with all components of the application, including a MassHealth application if you want to apply.  We’ll do our best to make the enrollment process easy and stress free. 

Step 4: Welcome to ESP!

Once enrollment is complete and you have been approved for Harbor Health’s PACE plan, services begin on the first of the following month. Shortly after you become a participant, you’ll get a personalized care plan created with input from you, your caregivers, and our care team to support your health, independence, and quality of life.

Harbor Health’s PACE plan delivers high-quality, personalized care to help older adults remain independent, active, and safe in their own homes and communities. By bringing together medical care, social support, and daily assistance, your care team works together to provide the care you need at home. 

To enroll in Harbor Health’s PACE plan, you or your loved one must live in or be moving into our service area. This ensures our care team can provide in-home services, transportation, and access to our ESP Day Centers.

Harbor Health’s ESP also collaborates local adult day centers as part of our effort to provide comprehensive services close to home. This collaboration allows our participants to receive a variety of services close nearby, fostering convenience and accessibility while enhancing their overall well-being.

Elder Service Plan Referral Form

Harbor Health Elder Service Plan (ESP) will be your sole provider for primary care and specialty care (other than emergency services). Care is provided directly by ESP or a provider we approve.
